Chronic Condition Compliance (CCC) Prescription Benefit
Well-managed chronic conditions result in improved health and lower costs for both you and DENSO.  Viverae’s Chronic Condition Management Program is available to provide you with support and information to better manage your condition.  Associates and spouses with certain conditions can save on the cost of their medications and supplies by partnering with the Viverae Chronic Condition Management program!

To qualify for discounts, you’ll need to take these steps:

1. Call the Viverae Health Center (1-888-848-3723) to set up a call with a coach.
2. During the call with your coach, the Viverae Health Clinician will review information about your condition and create a Care Plan (for each condition, if applicable), which is basically an action list for you to follow. If you created a Care Plan after January 1, 2014, you will not need to re-create one.
3. At any time after your Care Plan is created, you’ll need to call your coach to update him/her on your progress.

In order to qualify for 2015 CCC discounts, your coaching call will need to be on or after October 1, 2014.  Even if you have multiple Care Plans, only one follow-up coaching call is required to qualify for CCC. The CCC benefit will be in effect starting the first of the month following the completion of Step #3.
